Enrollment Information Updating
Are you an enrolled Suquamish tribal member who recently moved or legally changed your name? You’ve come to the right place to get your info updated.
To change your name or address with the Enrollment Department, complete the form below and attach a copy of two of the following documents (must be issued within the last two months):
- Utility service document (bill, statement, or hook-up order)
- Credit card statement
- Lease agreement
- Mortgage statement
- Homeowner’s or renter’s insurance policy
If you prefer, you may download the Change of Address Form and deliver it to the Suquamish Administration Building with your two supporting documents.
