Who is Eligible?
The Community Health Team provides services to all Suquamish Tribal Members and their families residing in Kitsap County. We also serve members of other federally recognized tribes, who reside within the Port Madison Indian Reservation.
What does Community Health do?
Our Community Health team uses non-clinical approaches for improving health and navigating barriers within the health system. We also focus on preventing disease and reducing health disparities. This is accomplished by addressing social, behavioral, environmental, economic, and medical factors throughout our community, its members, and their families.

Care Coordination
We organize patient appointments and care activities along with sharing information among everyone involved with a person’s care routine.
Medical Case Management
We coordinate with healthcare providers, manage appointments and treatment plans, provide certain nursing services, and help patients reach their optimal health.
Nurse Home Visits and Welfare Checks
These visits can include checking vitals, such as: blood pressure, temperature, heart rate, and breathing. The checks can also ensure that they’re taking their prescriptions, supplements, and performing any treatments correctly.
Patient Advocacy for Our Clients
As patient advocates, we give a voice to patients, survivors, and their caregivers by informing the public, healthcare providers, and other communities about any issues that arise. We also safeguard patients from errors, incompetence, and misconduct, protect patient rights, and build awareness and support for patients, survivors, and their caregivers.
Medical Transportation
Community Health offers assistance for anyone facing barriers that are preventing them from accessing transportation to their healthcare visits and/or medical related appointments.
Durable Medical Equipment Assistance, Coordination, and Delivery
Community Health can help you obtain, deliver, and in some cases, provide Durable Medical Equipment (DME) when needed. DME is any medical equipment used in the home. Examples are: Walkers, Wheelchairs, Oxygen Supplies, CPAP Machines, Glucose Monitors, and much more.
Nutrition Services
Community Health has a Registered Dietitian that can offer one-on-one or family consultations. Read more and get in touch here.
WIC Program
WIC stands for Women, Infants, and Children and it is a supplemental nutrition program for pregnant, breastfeeding or post-partum women, infants and children to age 5.
Support Groups
Community Health offers several support groups, and new ones are in the works. The topics include stress management, dealing with addictions, managing chronic diseases, general health, and diet. Reach out to us anytime for more information.
Educational Classes
We host classes in nutrition, chronic disease management, diabetes management, childbirth education, and more.
Tobacco Cessation Assistance
Community Health can help with the tools, support, and education needed to help someone quit their smoking habit.
Medication Management and Pick-Up
We oversee the usage of prescribed medications to ensure they are taken properly and achieve the desired outcome. This includes monitoring, reconciling medications, and reviewing prescribed drugs and their possible side effects to create treatment plans and monitor their safety and efficacy.
If you have any barriers preventing you from filling your prescriptions in person, we can have your medication picked up from your pharmacy and delivered to your door or assist you with enrolling in a home delivery plan.
Flu Vaccine Clinics
Community Health offers a Flu Vaccine Clinic where you can receive a vaccine that protects against the four influenza viruses that are most common during the Fall and Winter seasons. Protection from a flu shot lasts around 6 to 8 months. Once the season ends, the flu shot does not protect you any longer, so you will need a new vaccination for the following year’s season.
Covid-19 Vaccines and Boosters
People who are up to date on their COVID vaccines have lower risk of severe illness, hospitalization and death from COVID-19 than people who are unvaccinated or who have not completed the doses recommended for them by CDC. Additional updated COVID-19 vaccine boosters can help restore protection that has decreased since previous vaccination. Basic Nursing Services and so much more…
Community Health has nurses that fill health system gaps in our community. They go to organizations, schools, and businesses to offer health education, medical services, and rehabilitation. They can provide home health screenings, education, offer certain in-home treatments, and so much more.
